Overview

Our mental health programs enhance the quality of our clients’ lives by assisting them with achieving symptom management and goal attainment. Through the combination of group therapy and individualized treatment planning, each client can work on setting life goals in a therapeutic, caring and supportive environment.

Eligibility

In order to be eligible for our services, potential clients must meet the following criteria:

• Be at least 18 years old
• Have been diagnosed with a severe and persistent mental illness, such as schizophrenia or schizoaffective disorder. If you or a loved one have been diagnosed with a different persistent mental illness, please contact us for more information about eligibility and enrollment.
• Are not under ACT supervision
• Cannot attend an outpatient clinic concurrently with Continuing Day Treatment services.

Transportation

We are so dedicated to providing top quality services, when we were disappointed with the available options for transportation to and from scheduled services, we created our own solution.

GCC Transportation is a new service dedicated to providing people on Medicaid with reliable, personalized, friendly, and safe ground transportation to and from non-emergency medical services. We operate 24 hours a day, 7 days a week, 365 days a year. With our fully wheelchair-accessible vans, and drivers trained to provide personalized service to people with unique circumstances or conditions, we have a proven track record of transporting people to and from their medical services with almost no wait times.

GCC Transportation has a separate enrollment process than GCC Continuing Day Treatment Program, but we will assist you with both processes from beginning to end.

Meals

Meals for our clients are provided free of charge. Everyone who attends our services receive a morning breakfast, hot lunch, and afternoon snack. Our facility is also within short walking distance of a number of restaurants and bodegas.
